Never happened to me, but if it's a big enough organization I think it's possible.

Yes. They forgot I had interviewed previously though. And I didn't get the job the first time because of my geographic location. The second time it was not a factor.

I mean I made it to the final round, got told 'no', then was offered the job a month later

I've seen people get rejected, gain 1-2 years experience elsewhere, interview again and get the job. It would all depend on what you gained/learned since then.

Are we talking about Salesforce?  That’s a big enough company with so many departments/divisions and hiring managers that you could get hired 6 mos later.  Look into what their application cycle is or talk to the recruiter that managed the process to find out when you can submit another application at the company.
