It all depends. Are you sending the email through a verified domain? If yes is the DKIM correct? If no Authenticate the domain.
From their is it Transactional or is it Marketing style?
Transactional Email - Make sure you have opt in/ opt out, put the unsubscribed on. Shared IP with unverified emails to share the send score, warm up the IP on a dedicated IP if your trying to send a lot at once
Marketing style - Avoid buzzwords and links to unauthenticated sites to avoid the spam folder, bounces, and the promotional folders in Gmail, iCloud, and yahoo.

I think so
Hate that. If it's the whole domain, that goes above my skillset.
Try an email verification tool before sending? Maybe that answers your question. Give us some more details with what you are facing.
I'm being told by some people my email ends up in junk/spam and thats even sometimes direct from my email not CRM.
This could happen due to the graphics included in your email as well. Even pictures or logos in your signature.

Do you mean response rate or not having a return to sender? If its the later, zoominfo seems to have the correct email 99% of the time

Switch to a fax. No spam filter. Also takes you back to 1993.

I once got a bad list that I ran through outreach. Had a ton of bounces and it flagged my domain. If you've ran any bad lists through a sequence and received a lot of bounces I'd think you've been flagged.
Use an email verification tool, have an opt in/opt out option in your emails, be mindful of how many links you have in your emails.
send your cold email to mail-tester.com and see what it tells you. It's built for newsletters, but works well for cold emails too
