I believe Quality Assurance to be beneficial, sometime reps are unaware of some of the mistakes they make in a call, pinpointing these mistakes and showing them how to overcome them is the path to success! That's how you ramp up a rep!

I would think that there is always a fear of making a mistake and having a superior listen to it and get fired for it. Many of my peers have this fear, perhaps some of the Nays project that. As long as you don't insult the prospect, and are doing your best, such fear shouldn't exist. We won't know for sure until they speak up.

I use BANT as a foundation to start with. It def branches out into the other topics you refer to. BANT is not the be all end all but it should be covered in a meeting to qualify.
