Structure: 

1) Provide brief Description of Customer's role and company
2) Ask 3-4 calibrated questions about challenges/opportunities in the customers function or industry
3) Ask ONE question about how they use your product to address these challenges
4) Summarize 

Length: 
4-6 paragraphs total 

Quote: 
Ask yourself what's more impactful; the part about your product or the part about the problem it solves for(no right or wrong answer imo) 

Signature/pic/email of client?:
Pic, title, LinkedIn Bio/Link to Profile 


Do you write or let them write?:
Let them write, you edit for them, send for feedback/approval, revise, publish 

I'm sure people have success doing this differently. My 2 cents for what it's worth. Hope this is helpful!
