Alright, mine: Early days a BDR. I called a guy, got his secretary and she was like "nah, he won't be interested." Found a direct line and called. This guy was aware of my previous call and tore me an absolute new one and replied to a previous email to continue to tear me said new one.
Co-worker around the same time: She called, messes up her voicemail and goes "SHIT." She goes to start the voicemail over but that isn't an option on this particular system so she just had to leave that voicemail as it was.
