Not a hiring manager but this is what I always say.

Talk about a deal early in your role that your lost. You got the feedback you lost due to xyz, you incorporate it, leading to you hitting quota after learning from it.

The example I always use is I lost because I didnโ€™t understand the true pain the person was going through and I was showing the features. After learning from my mistakes, I do more research to understand the pain and how to quantify it

The manager is asking the interviewee about a deal that rep lost. So the question is, what does that manager want to hear from the interviewee about that deal?
That question comes up a lot in interviews, so OP is asking people who have been in the position to ask that question what they are interested in hearing.

Not a hiring manager, but I can tell you that they don't want to hear how it wasn't your fault that the deal was lost. They want to hear what you realize you could have done differently in hindsight, and how that has affected your sales process moving forward.
