Has anyone ever been fired for missing a flight to a work meeting due to issues out of their control? If so, what did you do about it? If not, what would you recommend doing about it.

Fired for missing a flight that was more than likely delayed due to the amazing airlines we have right now. Like @TennisandSalessaid, I think there is more to the story.

You can't help it if an airline cancels your flight. If you were late on your own accord that's a different story.
Are you an at will employee?
This would not be possible in my country. It’s extremely difficult to fire people. Tbh quite sad

That's a pretty petty company if that's it. I've been terminated before with no cause but found out a few months later it was due to budget cuts.
Missing flights happens. Sounds like a toxic employer. It's the cost/risk of doing business.
This may be the first time they've been burned and don't book refundable/rebookable tickets. It's worth every penny.

It’s easier than a lay off and more comfortable than a regular termination.
Companies will make all the excuses in the world for bad behavior IF you’re producing and they like you.
